• Make an appointment by clicking "Click to Book Appointment"

• Click the "Proceed" button for the appropriate consult.

• You will receive a text or email to log into the patient portal. All consent and intake forms are there for you to fill out PRIOR to your appointment for review and to meet federal guidelines.

• You will receive a text or email on the day of your visit with a link for secured video chat. *safari or chrome platform only*

• Lab work will be ordered and emailed to you after consultation.
